Nurse Care Manager

We are a collaborative, patient-centered primary care practice providing comprehensive care for individuals and families from birth through geriatrics. Our team is dedicated to promoting wellness, managing chronic conditions, and supporting patients in achieving their best health.

Position Summary
We are seeking a compassionate and experienced Registered Nurse (RN) / Care Manager to join our office-based primary care team. The ideal candidate will have experience in clinical care coordination, patient education, and managing both acute and chronic health conditions in an outpatient setting.

Main Responsibilities

  • Provide patient care and medical triage within the primary care office setting
  • Coordinate care across multiple providers and services
  • Educate patients and families on disease management, preventive care, and treatment plans
  • Administer vaccines and assist with office-based procedures
  • Support patients with behavioral health, substance use, and chronic disease management (such as diabetes)
  • Contribute to a collaborative team environment focused on quality care and patient satisfaction

Qualifications

  • Current Registered Nurse (RN) license (required)
  • Experience in a primary care or outpatient setting preferred
  • Strong communication and teaching skills
  • Ability to multitask and work collaboratively within a care team
  • Experience with care management, behavioral health, substance use, or diabetes education a plus

FAMILY NURSE PRACTITIONER OR PHYSICIAN ASSISTANT

FULL OR PART-TIME DENTAL HYGIENIST

WellOne Primary Medical and Dental Care is seeking a full-time or part-time (3 days/24 hours) dental hygienist to provide services at one of our three locations (Burrillville, Scituate, or North Kingstown, Rhode Island).  Applicants must be licensed in Rhode Island.  Previous experience is preferred but not required.
